What is the ACTion Team
The ACTion Team offers a variety of services and resources, including substance use and/or mental health treatment services. These services are designed to help individuals stay healthy and stable as they develop the skills needed to remain in the community and achieve their goals.
Who can participate?
The ACTion Team is primarily for individuals ages 14 and older who:
• have current or previous involvement in the criminal justice system,
• have a substance use or mental health issue, and
• live in Placer County.
What is expected of participants?
Participants are expected to be actively involved in the Team and work toward the goals identified in their Action Plan. All participants must agree to a background screening by Placer County Probation and will sign a basic information sharing agreement with the ACTion Team in order to provide the best support in accomplishing goals.
The ACTion Team is here to support you!
The ACTion Team is comprised of staff who have the skills and experience to help participants meet their life and health goals. This includes peer and family mentor advocates, counselors, case managers, and probation officers. The Action Team will work together with participants to celebrate successes and guide and support them to achieve health, wellness, and hope.
What services are available?
Plans and services are tailored to each participant’s strengths, needs, and preferences. They are designed to provide participants with accountability and support from the ACTion Team to accomplish their goals.
Services may include:
- Mental health and/or substance use disorder treatment
- Developing an Individualized Action Plan
- Enrolling in programs that help develop skills to reduce stress and substance use
- Attending school or training; finding and maintaining stable housing; learning new skills; gaining employment; developing a supportive network of friends, and more
- Engaging the families of participants to offer them support

The ACTion Team is funded through a Proposition 47 grant from the Board of State and Community Corrections. Granite Wellness Centers is the organization that is funded to collaborate with Placer County Probation and Health and Human Services to create a comprehensive support system for participants.

In collaboration with Granite Wellness Centers, the Placer County Probation Department, and the Placer County Department of Health and Human Services, is committed to ensuring a safe community by providing supportive services.
